RELEASE AND WAIVER OF LIABILITY

Japanese Martial Arts Academy, LLC / BDA Silvera Karate Jitsu

The undersigned participant (or parent/guardian if under 18) understands that participation in martial arts activities involves physical exertion, physical contact, and an inherent risk of injury, whether physical or emotional. I voluntarily assume full responsibility for any and all risks, injuries, or damages—known or unknown—that may arise from my or my child’s participation in any class, event, or training session at Japanese Martial Arts Academy, LLC / Silvera Karate Jitsu.

In consideration of being permitted to participate in a free introductory martial arts class, I hereby waive, release, and discharge any and all claims for damages due to personal injury, illness, property damage, or death against Japanese Martial Arts Academy, LLC, its owners, instructors, agents, and representatives, whether caused by negligence or otherwise.

I certify that the participant is physically capable of engaging in martial arts activities and has no known medical condition that would prevent safe participation. In the event of an emergency, I authorize the staff to seek appropriate medical care, and I agree to accept responsibility for any resulting medical expenses.

I also understand that photos or video footage may be taken during classes or events for promotional purposes.

By registering for a free introductory class, I acknowledge that I have read, understood, and voluntarily agree to the terms outlined in this Release and Waiver of Liability.
